True Choice Pathways is committed to providing respectful, person-centered, accessible services. Participants and families are encouraged to raise questions, concerns, or complaints.

Making a complaint will not negatively affect your services.

You may contact True Choice Pathways verbally or in writing regarding concerns about our services, communication, accessibility, privacy, professional conduct, billing, or other matters related to services we provide.

You do not need to use a special form. When possible, please tell us what happened, when it occurred, and what you would like us to consider doing to resolve the concern.

True Choice Pathways will review the concern, gather relevant information when necessary, consider appropriate corrective action or resolution, and communicate next steps or the outcome when appropriate.

Concerns will be handled as respectfully, privately, and promptly as reasonably possible.

True Choice Pathways prohibits retaliation against anyone who raises a good-faith concern, files a complaint, requests an accommodation, or seeks assistance from an outside agency or advocate.

If you need assistance communicating a concern, please let us know. Reasonable communication and accessibility assistance may be provided based on individual needs.

Using the True Choice Pathways complaint process does not prevent you from contacting San Diego Regional Center, the California Department of Developmental Services, an advocate, or another appropriate agency.

A DDS Individual Rights Complaint may be used when a person believes a regional center, state-operated facility, or service provider has violated or improperly denied a right. An appeal is generally used when the disagreement concerns eligibility or the nature, scope, or amount of regional-center services.

The True Choice Pathways complaint process is not a substitute for emergency assistance or mandatory reporting. Immediate health or safety concerns should be directed to the appropriate emergency or protective-service resource.
