True Choice Pathways helps participants and families understand their options, identify goals, organize information, develop person-centered plans, prepare for meetings, and navigate California’s Self-Determination Program (SDP).

Participants and their authorized representatives remain in control of their choices and final decisions. Selecting an Independent Facilitator is optional.

True Choice Pathways is an independent, privately owned business. We are not an employee, agent, or representative of the San Diego Regional Center, the California Department of Developmental Services (DDS), or an FMS provider.

For services provided through San Diego Regional Center:

Service Code 099 – Pre-Enrollment Self-Determination Support and Service Code 024 – Person-Centered Planning are provided only when authorized by the regional center.

Ongoing Independent Facilitation is separate from pre-enrollment services and, when selected by the participant, is generally included in the participant’s approved Self-Determination spending plan and paid through the participant’s FMS.

Rates, scope of service, and payment arrangements are discussed and agreed upon before services begin.

Regional centers, DDS, FMS providers, and other responsible entities retain authority over eligibility, individual budgets, service authorizations, spending plans, funding, and program approvals. True Choice Pathways cannot guarantee approvals, funding amounts, authorizations, timelines, or outcomes.

Services begin only after the required service agreement has been completed and, when applicable, written authorization or approved funding is in place.

True Choice Pathways does not provide emergency or crisis-response services.
