Personalized Self-Determination Support

Support for individuals and families preparing to enter the Self-Determination Program may include understanding the SDP process, organizing documents, preparing for meetings, exploring service and provider options, and assisting with budget and spending plan preparation. Person-centered planning is included throughout the process to keep the participant’s goals, preferences, priorities, and support needs at the center. 

  A collaborative process that identifies the participant’s strengths, interests, goals, preferences, support needs, and vision for the future. The process may include exploring natural and paid supports, identifying barriers, creating action steps, and preparing a written Person-Centered Plan.  

 Support is available to help participants and families understand the individual budget process, organize services around the participant’s goals, connect spending plan items to those goals, explore provider options, and coordinate with the Regional Center and FMS. 

 True Choice Pathways does not determine individual budgets or approve services. 

Participants enrolled in SDP may choose ongoing Independent Facilitation based on their needs and preferences. Support may include preparing for meetings, coordinating services and providers, communicating with the Regional Center and FMS, tracking goals and deadlines, and problem-solving when barriers arise.

Keep this sentence because it protects participant choice:

The participant decides what support is needed and how the Independent Facilitator will assist.

Services begin only after the necessary agreements, authorizations, and applicable approvals are in place. True Choice Pathways does not require private payment for services that are authorized and funded by the Regional Center.
