Mardex Enterprises LLC, doing business as True Choice Pathways (“True Choice Pathways,” “we,” “us,” or “our”), respects your privacy.

This Privacy Policy explains how we collect, use, disclose, retain, and protect information obtained through truechoicepathways.com, including Website forms, online scheduling, email, and telephone communications.

This Policy applies to Website inquiries, appointment scheduling, and general Website interactions. If you become a client, additional confidentiality, record-retention, authorization, and service-documentation requirements may apply.

1. Information We Collect

Information You Provide

When you contact us or schedule an appointment, we may collect:

  • Name; 
  • Email address; 
  • Telephone number; 
  • Preferred method of contact; 
  • Appointment date and time; 
  • Regional Center affiliation; 
  • Where you are in the Self-Determination Program; 
  • Whether SDP orientation has been completed; 
  • Type of service or assistance requested; 
  • Your relationship to the person seeking services; and 
  • Other information you voluntarily provide. 

Because our services relate to California’s Self-Determination Program, you may voluntarily provide information concerning disability-related needs, communication preferences, or Regional Center services. 

Please provide only the minimum information necessary for us to respond to your inquiry.

Do not submit Social Security numbers, financial-account information, passwords, government identification numbers, diagnoses, medical records, IPPs, IEPs, Regional Center records, conservatorship records, benefits records, or other highly sensitive documents through our Website forms, online scheduler, or ordinary email. 

If sensitive documents are needed, please contact us first so an appropriate method of transmission can be discussed.

2. Online Scheduling and Calendly

True Choice Pathways uses Calendly, a third-party scheduling service, to allow visitors to schedule consultations.

Calendly may process information such as your name, email address, telephone number, appointment preferences, and responses to scheduling questions. Calendly may also use cookies and similar technologies according to its own privacy and cookie practices.

Please do not include detailed medical information, diagnoses, Social Security numbers, UCI/client numbers, medical records, or other sensitive personal information in Calendly responses.

Scheduling or completing a consultation does not by itself establish a client or service relationship with True Choice Pathways.  

3. Information Collected Automatically

Our Website platform, security providers, Calendly, and analytics services may automatically collect technical information such as:

  • Browser and device type; 
  • Operating system; 
  • Pages viewed; 
  • Date and time of visits; 
  • Referring website or traffic source; 
  • Approximate geographic location; 
  • Cookie identifiers; and 
  • Website interactions and performance information. 

We use Google Analytics to understand general Website traffic and improve content, navigation, accessibility, security, and usability. We do not intentionally configure Google Analytics to collect names, contact information, medical information, appointment responses, Regional Center records, or other highly sensitive information. 

4. How We Use Information

We may use information to:

  • Respond to inquiries; 
  • Schedule and manage appointments; 
  • Prepare for consultations; 
  • Determine whether our services may be appropriate and available; 
  • Communicate about SDP-related services; 
  • Provide information about person-centered planning, transition support, spending plans, and Independent Facilitation; 
  • Respond to communication and accessibility requests; 
  • Maintain appropriate business records; 
  • Operate, secure, and improve our Website; 
  • Prevent fraud, misuse, or security incidents; 
  • Meet legal, contractual, insurance, tax, audit, or record-retention requirements; and 
  • Establish, exercise, or defend legal rights. 

We do not use Website or scheduling information to make automated decisions about eligibility for Regional Center services, public benefits, healthcare, employment, housing, education, credit, or insurance. 

5. Cookies and Analytics

Our Website and third-party services may use cookies and similar technologies.

Our Website may provide cookie controls allowing visitors to accept or decline optional analytics cookies. Declining optional analytics cookies does not prevent necessary Website, security, or scheduling functions from operating.

Calendly may provide its own cookie-preference controls within the embedded scheduling service.

We do not currently sell personal information or use Google Analytics for cross-context behavioral advertising or remarketing. 

If our tracking or advertising practices materially change, we will update this Policy and applicable cookie controls.

6. How We Share Information

We may share information only as reasonably necessary with:

  • Website hosting and scheduling providers, including GoDaddy and Calendly; 
  • Email, cloud-storage, IT, security, analytics, and business-service providers; 
  • Attorneys, accountants, insurers, auditors, and other professional advisers; 
  • Government agencies or other parties when disclosure is required by law, legal process, mandated-reporting requirements, or safety obligations; and 
  • Parties involved in a lawful business transaction. 

If you become a client, information may also be shared with Regional Centers, Financial Management Services providers, service providers, family members, representatives, or supporters when appropriately authorized or otherwise permitted by law.

Submitting a Website inquiry or scheduling a consultation does not authorize us to disclose your information to those parties. 

7. No Sale of Personal Information

True Choice Pathways does not sell personal information and does not currently share personal information for cross-context behavioral advertising.

We do not knowingly sell or share the personal information of minors. 

8. Retention and Security

We retain Website inquiries, appointment information, and related communications only as long as reasonably necessary for business, legal, contractual, tax, insurance, security, or recordkeeping purposes.

If an inquiry results in a client relationship, relevant information may become part of the participant’s service record and may be subject to separate retention requirements. 

We use reasonable administrative, technical, and physical safeguards designed to protect personal information. However, no Website, email system, scheduling platform, or electronic-storage system can be guaranteed to be completely secure.

General Website forms, Calendly, and ordinary email should not be used to transmit highly sensitive records. 

9. Children’s Privacy

Our Website is intended primarily for adults, including adults with disabilities, parents, family members, guardians, conservators, supporters, and authorized representatives.

The Website is not directed to children under 13. Children under 13 should not directly submit personal information through our Website or scheduling system.

A parent, guardian, or legally authorized adult may contact us concerning services for a minor. 

10. Third-Party Services

Our Website may contain or use third-party services, including:

  • GoDaddy; 
  • Calendly; 
  • Google Analytics; 
  • Regional Center and government websites; 
  • Financial Management Services providers; and 
  • Other community or service resources. 

These providers may have their own privacy policies, cookies, security practices, accessibility practices, and terms.

True Choice Pathways does not control independent third-party websites or services and is not responsible for their privacy, security, accessibility, or content. 

11. California Privacy Requests

California residents may contact us regarding personal information they have provided through our Website or scheduling system.

Depending on applicable law and the circumstances, you may request that we:

  • Confirm whether we maintain information about you; 
  • Explain what categories of information we collect; 
  • Provide access to certain information; 
  • Correct inaccurate information; or 
  • Delete certain information. 

We may need to verify your identity or authority before processing a request. Certain information may be retained when reasonably necessary for legal, contractual, tax, insurance, security, recordkeeping, fraud-prevention, or dispute-resolution purposes.

We will not unlawfully discriminate against anyone for making a privacy request.

This Policy does not state that True Choice Pathways is subject to every provision of the California Consumer Privacy Act. When an applicable California privacy law applies to us, we will comply with its requirements. 

12. Privacy Signals and Policy Updates

The Website does not currently change its practices in response to traditional Do Not Track browser settings.

Where applicable law requires us to recognize a qualifying browser-based privacy or opt-out signal, we will process it as required.

We may update this Privacy Policy when our Website, services, scheduling tools, analytics practices, technology, service providers, or legal requirements change. The revised Policy will be posted with an updated date.
